• Grassy Narrows & Islington Band Members are those that live on either Grassy Narrows First Nation (Asubpeeschoseewagong Netum Anishinabek) or Wabaseemoong Independent Nations, as well as band members who live off reserve.
• Because the effects of mercury poisoning are generational, blood relatives of Grassy Narrows & Islington Band Members who feel they are affected by mercury poisoning, may also  apply.
  • Applicants with no Band Number, and those that do not hold a Band Number for either Grassy Narrows or Wabaseemoong will be asked to also fill out the Additional Information Form with their application. This form will be submitted with your application (Assessment Request Form (ARF)) to your affiliated band for approval before it is returned to the MDB for processing.

All Child/Youth Applications (0-18 yrs. ) will require a parent or guardian to sign a Representative Form.

After your application has been processed:

• you will receive a letter and/or email from the MDB stating the status of your application. 

If Approved:

The MDB will make contact with you via phone and/or email to begin the next steps, which is
the Assessment Process.

Grassy Narrows Submission Option
